Office Policies
Missed Appointments
Our office policy requires that you give at least a 24 hour notice when canceling your appointment. If you No Call/ No Show you will be charged a $25.00, No Call/ No Show office visit fee. We do this to encourage our patients to keep their appointments so that should they need to cancel , we can accommodate other patients who may need to be seen by our doctors.
Co-Payments
If you have an insurance policy that requires a co-pay, this is due at TIME OF SERVICE. To accommodate you better our office is now equipped with a credit card terminal, however we will still honor cash and personal checks. We reserve the right to reschedule your appointment if co-pay is not received at time service is rendered.
Outstanding Balances
If you have a balance that is 90 days past due, our office now requires, BEFORE services are rendered, that you make a payment on your existing balance.
Please bring all updated information to your appointment. This includes your CURRENT address and phone number, CURRENT insurance card, photo ID, CURRENT list of medications and allergies, and name, address, and phone number of your primary care physician.
